ABSTRACT

Burnout encompassed a host of medical, psychological, emotional and performance-related issues that can always be affected by individual and coach factors in elite athletes;therefore, the aim of this study was to investigate the relationship between perfectionism and burnout, mediated by perceived coaching style in the context of the Covid-19 epidemic. In this cross-sectional study, 258 elite athletes (M = 22.05 years, SD = 3.08;148 male) participated and completed scales of multidimensional perfectionism, athlete burnout and coaching style. Data were analyzed using descriptive statistics by structural equation modeling .The results indicate that direct paths perfectionism to burnout, perfectionism to coaching style, coaching style to burnout, and indirect pathways to perfectionism to burnout were mediating by coaching style, significant. The results indicate that during the COVID 19 , athletes who are less able to organize performance standards and expectations of themselves and others, and who are more committed to meeting external demands, are at higher risk of burnout. One could develop coach-education programmes that challenge beliefs regarding the effectiveness of controlling coaching behaviors to reduce perfectionist tendencies and expectations in athletes. ABSTRACT

OBJECTIVES: To investigate possible persistent performance deficits after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in elite athletes. METHODS: A prospective cohort study in three Belgian professional male football teams was performed during the 2020 - 2021 season. Participants were submitted to strength, jump, and sprint tests and an aerobic performance test (the Yo-Yo Intermittent Recovery test (YYIR)). These tests were repeated at fixed time intervals throughout the season. Assessment of SARS-CoV-2 infection was performed by a polymerase chain reaction (PCR) test before each official game. RESULTS: Of the 84 included participants, 22 were infected with SARS-CoV-2 during follow-up. At the first testing after infection (52.0 ± 11.2 days after positive PCR testing) significantly higher percentages of maximal heart rate (%HRmax) were seen - within the isolated group of infected players- during (p = .006) and after the YYIR (2 min after, p = .013), compared to pre-infection data. This increase in %HRmax was resolved at the second YYIR testing after infection (127.6 ± 33.1 days after positive PCR testing). Additionally, when comparing the first test after infection in formerly infected to non-infected athletes, significantly higher %HRmax were found during (p < .001) and after the YYIR test (p < .001),No significant deficits were found for the jump, muscular strength or sprint tests.Aerobic performance seems compromised even weeks after infection. Simultaneously, anaerobic performance seemed to be spared. Because of the potential detrimental effects on the immune system, caution might be advised with high-intensity exposure until aerobic performance is restored.KEY MESSAGESElite football players' aerobic performance seems to be affected for weeks after they return to sports after a SARS-CoV-2 infection.Similarly, anaerobic performance tests showed no discernible changes between both before and after SARS-CoV-2 infections.Regular YYIR testing is recommended to monitor aerobic performance after SARS-CoV-2 infection.

ABSTRACT

BACKGROUND: Elite controllers are able to control viral replication without antiretroviral therapy. Exceptional elite controllers do not show disease progression for more than 25 years. Different mechanisms have been proposed and several elements of both innate and adaptive immunity are implicated. Vaccines are immune stimulating agents that can promote HIV-RNA transcription; transient plasma HIV-RNA detectability has been described within 7-14 days after different vaccinations. The most reliable mechanism involved in virosuppressed people living with HIV is a generalized inflammatory response that activates bystander cells harboring latent HIV. So far no data about viral load increase in elite controllers after SARS-CoV-2 vaccination are reported in literature. CASE PRESENTATION: We report the case of a 65-year-old woman of European ancestry, diagnosed with HIV-1/HCV co-infection more than 25 years ago. Since then, HIV-RNA remained undetectable and she never received ARV therapy. In 2021 she was vaccinated with mRNA-BNT162b2 vaccine (Pfizer-BioNTech®). She was administered with three doses in June, July and October 2021, respectively. The last available viral load was undetectable in March 2021. We observed an increase of VL at 32 cp/ml and 124 cp/mL, two and seven months after the second vaccine dose, respectively. During monthly follow-up, HIV-RNA gradually and spontaneously dropped becoming undetectable without ARV intervention. COVID-19 serology was positive with IgG 535 BAU/mL, showing response to vaccination. We measured total HIV-DNA at different time-points and we found it detectable both at the time of the higher plasma HIV-RNA (30 cp/10^6 PBMCs) and when it was undetectable (13 cp/10^6 PBMCs), in reduction. CONCLUSIONS: This case is the first report, to our knowledge, describing a rebound of plasma HIV-RNA in an elite controller after three doses of mRNA-BNT162b2 vaccine for SARS-CoV-2. Concomitantly with a spontaneous reduction of plasma HIV-RNA ten months after the third dose of mRNA-BNT162b2 vaccine (Pfizer-BioNTech®) without antiretroviral therapy intervention, we observed a reduction of total HIV-DNA in peripheral mononuclear cells. The potential role of vaccinations in altering HIV reservoir, even in elite controllers when plasma HIV-RNA is undetectable, could be a valuable aspect to take into account for the future HIV eradication interventions.

BACKGROUND: The COVID-19 pandemic has changed life styles of millions of people worldwide. This study investigated changes in the health, physical activity levels and eating habits of elite athletes during the COVID-19 pandemic lockdown in Iran. METHODS: 383 (248 female and 135 male) elite athletes (168.82 ± 0.07 cm; 63.92 ± 7.42 kg; the body mass index (BMI): 22.3 ± 0.78 kg/m2) participated in this study. The International Physical Activity Questionnaire (IPAQ), Depression Anxiety Stress Scales (DASS-21) and the Impact of Event Scale-Revised (IES-R) study tools were used to measure levels of physical activity and mental health status, respectively. The Emotional Eater Questionnaire (EEQ) was used to assess food consumption related to emotion. Pearson and Spearman correlation analysis test were used in data analysis. RESULTS: Levels of depression and stress were mild and moderate, while levels of anxiety were severe and very severe in most elite athletes. There were levels of low emotional eating by elite athletes during the COVID-19 pandemic. Physical activity levels were negatively correlated with psychological mood measures (p≤0.05), while there were positive correlations between emotional eating behaviours and psychological mood measures (moderate correlation; p≤0.01) and light physical activity levels (weak correlation; p≤0.05). CONCLUSION: This study provides the first preliminary evidence showing that the COVID-19 lockdown conditions negatively influenced the eating habits and levels of physical activity and mental health in elite athletes. Regular high intensity physical activity as health strategy in elite athletes and the general population remains a strategy to improve overall health during the COVID-19 pandemic. Additionally, these findings suggest the need to devise strategies to improve the life styles of elite athletes during pandemics such as the Covid-19 pandemic.

Cette étude a examiné certains indicateurs de bonne santé, les niveaux d'activité physique et les habitudes alimentaires d'athlètes Elite iraniens pendant le confinement lié à la pandémie de COVID-19.Méthodes : 383 (248 femmes et 135 hommes) athlètes élites (168,82 ± 0,07 cm ; 63,92 ± 7,42 kg ; indice de masse corporelle (IMC) : 22,3 ± 0,78 kg/m2) ont participé à cette étude. Le questionnaire international sur l'activité physique (IPAQ), l'échelle DASS-21 (Depression Anxiety Stress Scales) et les scores de l'auto-questionnaire Impact of Event Scale-Revised (IES-R) ont été utilisés pour mesurer les niveaux respectifs d'activité physique et de l'état de santé mentale. L'Emotional Eater Questionnaire (EEQ) a été utilisé pour évaluer les altérations de la prise alimentaire liées à l'émotion. Le test d'analyse de corrélation de Pearson et Spearman a été utilisé pour l'analyse des données.Résultats : Les niveaux de dépression et de stress étaient légers et modérés, tandis que les niveaux d'anxiété étaient sévères à très sévères chez la plupart des athlètes élites. Il y avait des niveaux de faible alimentation d'origine émotionnelle chez ces athlètes pendant la pandémie de COVID-19. Les niveaux d'activité physique étaient corrélés négativement avec les mesures de l'humeur psychologique (p≤0,05), tandis qu'il y avait des corrélations positives entre les comportements alimentaires émotionnels et les mesures de l'humeur psychologique (corrélation modérée ; p≤0,01) et les niveaux d'activité physique légère (faible corrélation ; p≤0,05). CONCLUSION: Cette étude apporte une preuve préliminaire que les conditions de confinement liées au COVID-19 ont influencé négativement les habitudes alimentaires et les niveaux d'activité physique et de santé mentale chez des athlètes élites. L'activité physique régulière à haute intensité aussi bien chez les athlètes élites que pour la population générale reste une stratégie pour améliorer la santé globale pendant la pandémie de COVID-19. De plus, ces résultats suggèrent la nécessité de concevoir des stratégies pour améliorer les styles de vie des athlètes élites lors de pandémies, telles que celle de Covid-19.

Background/Aims The impact of COVID-19 has not been previously documented in an elite sporting population. Previous reports have documented cardiorespiratory sequelae in athletes, and expert opinion-based guidance is generally employed in their rehabilitation. However, there still persists a lack of information on how COVID-19 affects elite athletes, and a standardised approach to graduated return to play (GRTP). This case series describes a cohort of six elite Irish-based athletes diagnosed with COVID-19. Methods Six athletes were followed from symptom onset until their GRTP. A retrospective case series was performed using a standardised audit form for COVID-19 among elite athletes. Results Headache and fatigue (6/6) were the commonest presenting symptoms, with mean duration 4.5 days;all athletes experienced mild illness, none required further investigation. 3/6 athletes supplemented vitamin D. Mean rest period was 11.5 days and mean GRTP 19.2 days. All athletes had an unremarkable screening electrocardiogram. Conclusion This case series presents the first clinical summary of COVID-19 illness in elite athletes based in Ireland. Treatment is largely supportive although further consideration should be given to implementing supplementation, such as vitamin D, in the management of COVID- 19. Larger, longer term studies are required to better understand the impact of COVID-19 in athletes and across different sports.

ABSTRACT

History: A 20 year old D1 men's basketball player with a history of COVID the month prior presented with worsening low back pain. He denied any injury, but reported the pain started as low back discomfort after a basketball game the week prior. He noted a progression and radiation of pain down his right lower extremity to his toes. He had tried physical therapy and dry needling, as well as cyclobenzaprine and naproxen from team physicians with mild improvement. The pain worsened and he went to the ED for evaluation. He was afebrile and had a lumbar radiograph with no acute fracture, grade 1 anterolisthesis of L5 on S1. He was discharged home with norco. Over the next 2 days, he developed chills and in the context of his worsening back pain, his team physicians ordered an MRI. Physical Exam: BMI 26.9 Temp 97.9degree Heart rate: 73 Respiratory rate 14 BP: 124/64 MSK: Spine- Intact skin with generalized pain over lumbar area, worse over the right paraspinal musculature. 5/5 strength of bilateral lower extremity flexion and extension of his hips, knees, and plantar and dorsiflexion of ankles and toes. Bilateral intact sensibility in the sciatic, femoral, superficial, and deep peroneal, sural, and saphenous nerve distributions. Slightly diminished sensibility over the right deep peroneal nerve distribution compared to left. 2/4 patellar and achilles DTRs. No clonus, downgoing Babinski sign. Positive straight leg raise at 45 degrees with the right lower extremity. Differential Diagnosis: 141. Sciatica 142. Lumbar Muscle Strain 143. Disk Herniation 144. Spondylolisthesis 145. Vertebral Osteomyelitis Test Results: CBC:WBC10, HGB13.2, neutrophils 75.7% (red 45%-74%). Unremarkable CMP. CRP =7.31, ESR 23 Blood culture negative, throat culture negative. TB test negative. COVID test negative. Flu test negative. Urine culture and UDS negative. HIV test negative. Procalcitonin of 0.07. IR guided aspiration and bacterial Culture yielded MSSA. MRI w/contrast: showing L1-L4 facet edema concerning for infectious spondylitis, intramuscular, and epidural abscess. Final Diagnosis: Acute intramuscular abscess, vertebral osteomyelitis, with epidural abscess. Discussion(s): Vertebral osteomyelitis is a serious but quite rare disease in the immunocompetent, elite athlete population. Staphylococcus Aureus is the culprit a majority of the time, with only 50% of cases showing neurologic symptoms. This case was unique given the proximity to a dry needling treatment which is the only explainable vector of infection, normal blood cultures in this disease which hematogenously spreads, negativeHIV and other infectious disease testing, and otherwise benign history. Early recognition of this disease yields better outcomes and reduces incidence of severe debility. 5% to 10%of patients experience recurrence of back pain or osteomyelitis later on in life. Outcome(s): Patient was hospitalized and started on Cefepime and Vancomycin. Had an echocardiogram revealing changes consistent with athlete's heart without signs of vegetation on his cardiac valves. Neurosurgery declined to treat surgically. He continued to improve until he was ultimately discharged on hospital day 4 with a picc line and Nafcillin and was later changed to oral augmentin per ID. Follow-Up: By his 6 week follow-up visit with infectious disease and the team physicians, his back pain had completely resolved and was cleared to start a return to play protocol. There was no progression of disease since starting antibiotics, and no recurrence of back pain since treatment.

ABSTRACT

Aim of the Study: We aimed to evaluate the virus spreading among a migrant population previously excluded by community surveillance programs. Method(s): We conducted a retrospective study, collecting data about people without SARS-CoV-2-related symptoms who attended the outpatient clinic for undocumented migrants from November 1, 2020, to April 30, 2021. Patients who performed a nasopharyngeal swab and serologic test to evaluate the presence of antibody anti-SARS-CoV-2 were enrolled. Result(s): Overall, 240 people were included in our study. Of them, 15 (6.3%) were female, with a median age of 27.0 years (interquartile range [IQR], 24.3-32.1 years). Thirty-seven patients (15.4%) tested positive for SARS-CoV-2 at the nasopharyngeal swab during the study period. Of these, 16 had positive or low positive results for immunoglobulin G (IgG) and 3 tested positive for both IgG and IgM. Besides, 22 participants (9.2%) resulted positive to serological testing, but negative to polymerase chain reaction testing. The median age of SARS-CoV-2 positive patients (n = 59) was significantly higher than negative (29.6 [IQR, 25.0-35.0] vs 26.8 [IQR, 24.2-31.5], P = 0.022). Among positive patients, the most frequent nationality was Bangladeshi, with 24 people (40.7%, P < 0.001). The highest percentage of positive was found among the same nationality (51.1% of Bangladeshi tested positive). Conclusion(s): Our data underline the significantly higher prevalence of SARS-CoV-2 infection in the undocumented migrant population in respect of the general population of Piacenza province in the same period (15.4% vs 5.9%, P < 0.001). The extension of surveillance programs to the whole population, thus including undocumented people, is crucial to curb the spreading of the virus.Copyright © 2022 Wolters Kluwer Health, Inc. ABSTRACT

Understanding population-level variability in responses to pathogens over time is important for developing effective health-based messages targeted at ideologically diverse populations. Research from psychological and political sciences suggests that party and elite cues shape how people respond to major threats like climate change. Research on responses to the COVID-19 pandemic suggests similar variability across party identities;however, prior work has methodological limitations. This prospective, longitudinal study of a large probability-based nationally representative U.S. sample assessed in March–April 2020 (N = 6,514) and then 6 months later in September–October 2020 (N = 5,661) demonstrates that COVID-19 fear, perceived COVID-19 death risk, and reported health-protective behaviors became increasingly polarized over the first 6 months of the pandemic. Initial differences between Democrats and Republicans failed to converge over time and became more pronounced. Responses among Republicans were further polarized by support for former President Donald Trump: Trump Republicans initially reported weaker responses to COVID-19 than non-Trump Republicans, and these differences became more pronounced over time. Importantly, political identity and Trump support were not linked to perceived infection risk of a nonpoliticized pathogen, the flu. Finally, political identity and Republican Trump support prospectively predicted COVID-19 vaccine intentions 6 months into the pandemic. © 2023 International Society of Political Psychology.

ABSTRACT

BACKGROUND: Some probiotics appear to improve athletic performance, endurance, and recovery after intense exercise. Other formulations may provide performance-related benefits via immune and gastrointestinal functions in athletic individuals. However, few formulations have been studied for both types of effects among non-elite athletes. The primary objective of this study is to assess the ergogenic effects of a probiotic on high-intensity endurance running performance in non-elite runners. Secondary objectives include assessment of perceived exertion, blood chemistry, immune and stress biomarkers, cold and flu symptoms, and gastrointestinal health after the probiotic intervention. METHODS: This 9-week randomized, placebo-controlled, double-blind, parallel trial will assess the ergogenic effects of a probiotic (5 billion colony-forming units/day, for 6 weeks) in healthy, non-elite runners (N=32; 18-45 years). Participants will be monitored via daily and weekly questionnaires during the 2-week pre-baseline, 6-week intervention, and 1-week washout. Questionnaires will inquire about activity, muscle soreness, gastrointestinal symptoms, cold and flu symptoms, stool form and frequency, and adverse events. During the pre-baseline visit, maximal oxygen uptake (V̇O2 max) is assessed to set appropriate individualized workload settings for the treadmill time-to-exhaustion endurance tests. These time-to-exhaustion endurance running tests will be completed at an intensity of 85% VO2max at baseline and final visits. During these tests, self-perceived exercise effort will be rated via the Borg Rating of Perceived Exertion scale and finger sticks assessing capillary blood glucose and lactate concentrations will be collected every 3 min. Additional questionnaires will assess diet and motivation to exercise. Body composition will be assessed using air displacement plethysmography at the baseline and final visits. Hypotheses will be tested using two-sided tests, and a linear model and with a type I error rate of α=0.05. Primary and secondary outcomes will be tested by comparing results between the intervention groups, adjusting for baseline values. DISCUSSION: These results will build evidence documenting the role of probiotics on running endurance performance and physiological responses to exercise in non-elite athletes. Understanding the potential mechanisms of probiotic effects and how they mitigate the intestinal or immune discomforts caused by running could provide additional strategy means to help runners improve their performance. ABSTRACT

This commentary offers an analysis of the implications of Covid-19 on the future of elite women's football, with the intention of reflecting on and illuminating the threat and uncertainty facing the game. Topics covered include (1) organisational and economic repercussions;(2) consequences for player contracts, migration and investment;and (3) player wellbeing. These significant challenges require swift and decisive action in order to mitigate their potential effects. Recommendations are made for governing bodies, parent clubs and practitioners, in addition to possible future research directions for academics. We reflect upon what can be done during and post-pandemic to continue the momentum and growth of women's football in England, highlighting the need for football clubs to learn from previous crises by embracing innovation and entrepreneurship.

ABSTRACT

The Covid-19 pandemic has had huge ramifications on professional football. This commentary focuses on the impact of the pandemic on the mental health of professional footballers. Specifically, footballers within the English Premier League, English Football League, FA Women's Super League and FA Women's Championship. This commentary considers a holistic approach to mental health, the environment of professional football, and the impact of career transitions and critical moments on mental health. The intention is to stimulate discussion and further research of mental health and wellbeing within professional football. This paper considers the impact of Covid-19 and makes recommendations for professional football clubs to develop a holistic mental health strategy. We recommend that professional clubs increase the level of emotional support for professional footballers, and that this should not be a temporary measure due to the pandemic. Clubs should develop a long-term strategy to encourage players to seek emotional support.

COVID-19 vaccination raised concerns about its potential effects on physical performance. To assess the impact of COVID-19 vaccination on the perceived change in physical performance, we conducted an online survey among elite athletes from Belgium, Canada, France and Luxembourg, with questions about socio-demographics, COVID-19 vaccination, perceived impact on physical performance and perceived pressure to get vaccinated. Full vaccination was defined as two doses of mRNA or vector vaccine or a heterologous vaccine scheme. Among 1106 eligible athletes contacted, 306 athletes answered the survey and were included in this study. Of these, 72% perceived no change in their physical performance, 4% an improvement and 24% a negative impact following full COVID-19 vaccination. For 82% of the included athletes, the duration of the negative vaccine reactions was ≤3 days. After adjustment for potential confounding variables, practicing an individual sport, a duration of vaccine reactions longer than 3 days, a high level of vaccine reaction and the perceived pressure to get vaccinated were independently associated with a perceived negative impact on physical performance of more than 3 days after the vaccination. The perceived pressure to get vaccinated appears to be a parameter associated with the negative perceived change in the physical performance and deserves further consideration.

The authors operated in professional roles related to preparations for Tokyo 2020 when the COVID-19 pandemic changed the world. The purpose of this article is to share applied experiences and reflections from different parts of the world of psychological challenges and support strategies adopted in elite sports during the first phase of the pandemic (i.e., March to August 2020). Common themes included motivational issues, stress and anxiety, loss of routine, life-balance challenges among athletes, but also benefits of extended preparation time and recovery. Introduction: The media coverage of high-profile sporting events can bring increased pressure to athletes' psychological wellbeing. There may be speculation regarding athletes' mental state before, during and after competition potentially impacting on both the athlete's wellbeing and public perception of the individual if a negative tone is attached to an article. As mental health understanding and literacy develop, media reporting of elite athlete mental health may contribute to shaping of opinions and help seeking behaviour. Thus, we have aimed to understand and analyse the tone and content of media reporting on a high-profile sporting event using qualitative methods to explore key aspects of the articles sampled. Methods: We selected two UK broadsheet newspapers available online (The Guardian and The Telegraph) and identified all text articles including the terms "Mental Health" and "Olympics" or "Paralympics" published between 23rd July 2019 and 30th November 2021. We selected articles relevant to the Tokyo 2020 Olympic and Paralympic Games and including mental health as a subject. Relevant articles were read in full, and we conducted a thematic analysis to explore how mental health was portrayed. Results: 581 results were generated from our initial search. Following screening and removal of irrelevant articles, we had 95 articles for analysis. We identified four themes relating to portrayal of mental health: Pressure, impact of COVID, Lack of specific diagnostic terms, and athletes as humans. Conclusion: Media portrayal of mental health at the Tokyo Games was generally positive and portrayed athletes as relatable, inspiring, and "human" rather than superhuman. Reporting particularly focused on athlete-specific factors in mental health conditions and the pressures of high-level competition. (PsycInfo Database Record (c) 2022 APA, all rights reserved)

ABSTRACT

Saudi Arabia's Public Investment Fund (PIF) has grown from marginal player to the most important economic actor in the Kingdom since 2015. Nevertheless, we know surprisingly little about the political economy of the PIF revamp. Against an unfavorable macroeconomic backdrop, I argue that shifts in PIF organization and orientation are fundamentally driven by the centralization of power within the circles of the Saudi ruling family since the rise of Mohammed bin Salman (MBS). The fund's governance framework and network of insiders forming the board of directors mirror the concentration of authority around MBS. In turn, PIF domestic activities shifted from scattered investment patterns associated with the competing influence of senior decision-makers to a highly authoritative and personalized strategy. Moreover, the PIF's response to COVID-19 further exemplifies the turn from a conservative strategy toward a short-term oriented approach to sovereign wealth management. Going beyond macro-level economic and institutional dynamics, the article introduces the role of political agency in SWF choices by stressing how political actors and the distribution of power within ruling elites shape SWFs. The article thus adds to the scholarship on domestic drivers of SWFs and contributes to debates surrounding the interplay between states and markets under processes of financialization.

ABSTRACT

The emotional dimensions of a coach's activity have been few analyzed in sport psychology. However, an elite coach usually experiences intense emotions requiring real emotional labor, which can influence the daily activity. The notion of emotional labor, stemming from sociology, is increasingly used in a psychological setting to understand the impact of emotions on the coach's activity. Based on a collaboration of three consecutive seasons during the Covid-19 pandemic with an elite handball coach, this article formulates recommendations for revaluing the place of emotional labor in the practice of sport psychology. © 2023 Association for Applied Sport Psychology.

ABSTRACT

Purpose: We evaluated the extent of changes in training practices, recovery, mental health, and sleep patterns of athletes during the early COVID-19 lockdown in a single country-cohort. Methods: A total of 686 athletes (59% male, 41% female; 9% World Class, 28% International, 29% National, 26% State, 8% Recreational) from 50 sports (45% individual, 55% team) in Malaysia completed an online, survey-based questionnaire study. The questions were related to training practices (including recovery and injury), mental health, and sleep patterns. Results: Relative to pre-lockdown, training intensity (-34%), frequency (-20%, except World-Class), and duration (-24%-59%, especially International/World-Class) were compromised, by the mandated lockdown. During the lockdown, more space/access (69%) and equipment (69%) were available for cardiorespiratory training, than technical and strength; and these resources favoured World-Class athletes. Most athletes trained for general strength/health (88%) and muscular endurance (71%); and some used innovative/digital training tools (World-Class 48% vs. lower classification-levels ≤34%). More World-Class, International, and National athletes performed strength training, plyometrics, and sport-specific technical skills with proper equipment, than State/Recreational athletes. More females (42%) sourced training materials from social media than males (29%). Some athletes (38%) performed injury prevention exercises; 18% had mild injuries (knees 29%, ankles 26%), and 18% received a medical diagnosis (International 31%). Lower-level athletes (e.g., State 44%) disclosed that they were mentally more vulnerable; and felt more anxious (36% vs. higher-levels 14%-21%). Sleep quality and quantity were "normal" (49% for both), "improved" (35% and 27%), and only 16% and 14% (respectively) stated "worsened" sleep. Conclusion: Lockdown compromised training-related practices, especially in lower-level athletes. Athletes are in need of assistance with training, and tools to cope with anxiety that should be tailored to individual country requirements during lockdown situations. In particular, goal-driven (even if it is at home) fitness training, psychological, financial, and lifestyle support can be provided to reduce the difficulties associated with lockdowns. Policies and guidelines that facilitate athletes (of all levels) to train regularly during the lockdown should be developed.

Introduction: The cardiovascular benefits of physical exercise are well-known. However, vigorous exercise has also been associated with adverse cardiac effects. To improve our understanding of cardiovascular adaptation to exercise versus maladaptation and pathology, the limits of adaptation should be firmly established using state-of-the-art diagnostic modalities. We therefore initiated the Evaluation of Lifetime participation in Intensive Top-level sports and Exercise (ELITE) cohort to investigate the longitudinal (beneficial and pathological) cardiovascular effects of intensive elite sports and exercise. Methods and analysis: ELITE is a prospective, multicentre, longitudinal cohort study. Elite athletes, from the age of sixteen, are recruited in The Netherlands. The primary objective is to determine the association between elite sports and exercise-induced cardiac remodelling, cardiac pathology, and health benefits over time. Secondary objectives include determining and identifying genetic profiles of elite athletes, and how these are associated with cardiac indices. ELITE will collect data from consultations, electrocardiography, echocardiography and cardiac magnetic resonance imaging, and training- and injury data. ELITE will also collect blood for biobanking and cardiogenetics. Follow-up will take place at intervals of two to five years, and after the elite athletes' professional careers have ended. In addition, a subcohort of ELITE has been established to investigate cardiac sequelae following infections associated with myocardial involvement, including SARS-CoV-2. ELITE is a prospective observational study; therefore, analyses will be primarily explorative. Ethics and dissemination: This study has been approved by the Medical Ethics Review Board of the Amsterdam University Medical Centers (NL71682.018.19). The results of the study will be disseminated by publication in peer-reviewed journals (Netherlands Trial Register number: NL9328).

ABSTRACT

The incidence and risk factors for poor outcome in patients with COVID are well known, as are the protective measures in high-risk populations. In contrast, the epidemiological and clinical behavior of this disease in the population of elite athletes who are the paradigm of good health is poorly understood. Data on COVID in athletes are scarce and have focused preferentially on the consequences on sports performance of confinement measures and on the pathophysiological risks of infected athletes. Real Madrid is a large elite sports institution with facilities in the City of Madrid where 600 athletes train daily. The incidence of COVID during a study period of 671 days of observation in athletes, professional or amateur, working in the institution has been 0,74 per 1,000 days of exposure. The disease has been asymptomatic or oligosymptomatic in all athletes and did not require any hospital admissions. The different teams did not have to suspend any of its sportive commitments for COVID during the study period and there was no evidence of outbreaks of internal transmission between members of the different teams.

ABSTRACT

COVID-19 diagnosis has become a crucial task in today's world due to the rapid spread of the infectious Corona Virus disease caused by the SARS-CoV-2 virus. Analysis of COVID using CT scan images is shown to give better results but it requires expert radiologists and it consumes time. Hence there is a need for a diagnosis system to classify whether it's COVID positive or not for quick and early diagnosis. Deep Learning models are effective in handling classification problems but some models might lead to vanishing gradient problem. A Mixture Density Network (i.e.) Bidirectional Long Short-Term Memory((Bi-LSTM) with Mixture Network is used as the classifier to handle the vanishing gradient problem and to classify based on the probability distribution. Parameter tuning plays a major role in improving the overall efficiency of the classifier. An Enhanced Memetic Adaptive Differential Evolution (EMADE) algorithm is proposed for tuning the parameters of the classifier. Enhanced MADE is a memetic algorithm with proposed Elite chaotic local search (ECLS) which helps in addressing the issue of getting stuck at a local optimal solution and premature convergence. The use of Elitism in the chaotic local search directs the algorithm toward the optimal solution and increases the exploitation ability. Due to high false negatives in RT-PCR, CT scan images have been taken as the input. The dataset is labeled and it consists of 1252 CT scans that are positive for COVID-19, and 1230 CT scans that are negative for COVID-19. The dataset collected from patients in Sao Paulo, Brazil that is available on Kaggle is used [21]. A sample of the dataset is taken for experimentation and an accuracy of 75.83% is achieved. The precision is 80.32% indicating that there are fewer False positive than the existing methods. © 2022 IEEE.
